Terms of Engagement

DR SUNNI PATEL PHD MBA BSC (HONS) AND REGISTERED NUTRITIONAL/CULINARY MEDICINE COACH

Please read the following terms and conditions carefully.


NUTRITIONAL COACHING APPROACH


Nutritional Coaching is the application of nutrition science in the promotion of health, peak performance and individual care. Coaching uses a wide range of tools to assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows us to address nutritional balance and help support the body towards maintaining health. Culinary Medicine is recognized as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing.


Practitioners consider everyone to be unique and recommend personalized nutrition and lifestyle programmes rather than a ‘one size fits all’ approach. Practitioners never recommend Nutritional Coaching as a replacement for medical advice and always refer any client with ‘red flag’ signs or symptoms to their medical professional. They may also frequently work alongside a medical professional and will communicate with other healthcare professionals involved in the client’s care to explain relevant Nutritional Coaching interventions.


1. Nutritional Coaching works alongside conventional forms of care. Nutritional Coaching is not intended to replace medical advice, diagnose or treat medical conditions. It is your responsibility to inform your doctor of all medical concerns. You are also advised to keep your doctor informed of the recommendations you receive from this coaching because of possible interactions between medications and your nutritional programme. Nutritional advice is not a substitute for medical advice and/or treatment.


2. Dr Sunni requires that you provide full contact details of your GP. Should your practitioner identify a need to contact your GP about an aspect of your health or medical history, a letter will be sent directly to your GP. You will be informed and consulted about all procedures relevant to your case.


3. The suitability of the advice you are given depends considerably on the accuracy of the information you provide in the questionnaires and during consultation. No responsibility can be accepted in the case of omitted or inaccurate information. You are expected to provide complete details of diagnosed medical conditions as well as any medication(s) being taken. Changes in diagnosis and/or medication during your therapeutic programme with Dr Sunni should be informed as soon as possible. You are also expected to provide full details of other nutritional supplements, herbal remedies and other therapeutic agents you may use during your therapeutic programme.


4. Payment is taken in advance of the starting the coaching programme. You should contact Dr Sunni for details on how to pay by BACS and invoicing. Each session has an agreed time slot so that the end time cannot be changed. Please note that a late start will result in a shorter session and no refund will be issued.


5. Cancellations or rescheduling must be communicated at least 48 hours in advance or session still maybe charged to the client at full price. Failed attendance or late cancellation will result in the session to be lost or still be charged to the client.


6. The Preliminary Assessment Questionnaires and the Food Diary are documents made available to you when you book your first appointment. They should be completed and submitted to Dr Sunni at least one week prior to the initial consultation. Failure to send this information in time may result in the consultation being postponed. These documents are essential tools used in the buildup of your case and analysis of your symptoms and conditions. They save valuable time of your personal consultation and provide information on which the practitioner starts working even before you meet for the first appointment. It is important that all information provided is as accurate as possible.


7. Communication between consultations aims to address unexpected reactions that require urgent attention. Dr Sunni will advise you on arranging a virtual or telephone consultation between visits in order to provide further support if needed. Please note it is not possible to provide advice via e-mail. You are welcome to e-mail your queries, but please expect to be offered a time slot for an extra phone or virtual consultation to discuss them.

8. When necessary, laboratory tests may be suggested as tools to clarify possible underlying imbalances. These tests incur additional costs although some might be available through NHS if agreed by your GP. Dr Sunni will explain the applicability of possible tests giving you the choice to take them or not. You will be always involved in the decision-making process of your programme. Test results require professional interpretation and for that reason are ordered and sent directly to your GP.


9. The outcomes of lifestyle coaching vary amongst individuals and the success of the programme will depend on your compliance. The Client is 100% committed to the programme. And your commitment to the programme will dictate the success of your journey. Coaching puts you, the client, in the driving seat. The results and the implementation of your agreed action plans are your responsibility. The client agrees to always be honest and to feedback promptly and openly if any problems arise during the sessions. This is applicable to any practical concern about coaching or concerns about something the coach may have said. No responsibility can be accepted in the case of non-compliance, alterations to the original programme by third parties or alterations to any supplement suggestions. If you are unclear about the agreed programme/food supplement doses/time period, you should contact Dr Sunni promptly for clarification.


10. All individual information gathered and analysed in the process of a case is strictly confidential. The information you provide Dr Sunni will be safely processed and stored for the purposes of the development of your therapeutic programme, administration and future reference. Dr Sunni has agreed to the Code of Ethics and recognises that anything the client shares during the sessions are completely confidential. Dr Sunni therefore agrees to never disclose any personal details directly or indirectly unless required by law. Dish Dash Deets complies with the Data Protection Act 1998 and your details will not be passed on to third parties without written consent. Dr Sunni will always comply with GDPR and maintain his professional indemnity insurance.


11. Recording consultations using any form of electronic media is not allowed without the written permission of both client and Dr Sunni.


12. Programme sessions must be used within 4 weeks of programme end date. Only plans longer than 3 months can be put on “on  freeze” for a maximum of 1 month.

Coaching Framework and Service Agreement

This section outlines the Gut Health and Wellness coaching framework. The coaching program is holistic in nature primarily centred around gut health, plant-based food and lifestyle. The approach aims to empower the client to make healthier long-term decisions to support good gut health and overall positive health outcomes.


The health coaching process will encourage reflection, self-awareness and education, self-regulation and positivity to provide
you with the advice, experience and tools to feel empowered towards better gut health. The program aims to build confidence, focusing on strengths as they learn from past failures and successes to overcome current issues around your health.


This is a 12-week program which can be extended after completion and requires total commitment to the co-created goals and plan of action to ensure appropriate monitoring of results.


The 12 weeks entail an initial 60 minute 1:1 to understand goals, needs and issues followed by another 60 minute 1:1 to present a report of recommendations and suggested meal plans and recipes supported by lifestyle planning (stress management, exercise, mindfulness). All sessions will involve in-depth evidence and science-backed information. Thereafter there will be weekly 60 minute 1:1 to monitor progress and adapt any coaching plans where necessary with a final review and next steps session to close the program. A weekly session form will be provided to complete in advance of the 1:1.


To outline what is included as part of the 12-week Program:
• 1 x 1:1 Consultation Meeting at a predefined time
• 1 x 1:1 Report and Goals Session with Meal (and Exercise) Plans at predefined times
• 10 x 1:1 Weekly Touchpoint Sessions at predefined times
• 1 x 1:1 Final Review Session at a predefined time
• Access to learning materials (Deets Sheets)
Other additional requirements such as cook-a-longs, exercise sessions, meal provision, shopping support and expert sessions
form part of the VIP Membership and a quote can be created on request.


The total cost of the 12-week program is £1200 which should be paid in advance of commencing the program. Should you require a payment plan please enquire to get details.

Pro Bono Coaching Framework and Service Agreement

This section outlines the Gut Health and Wellness coaching framework(4 week). The coaching program is holistic in nature primarily centred around gut health, plant-based food and lifestyle. The approach aims to empower the client to make healthier long-term decisions to support good gut health and overall positive health outcomes. The health coaching process will encourage reflection, self-awareness and education, self-regulation and positivity to provide you with the advice, experience and tools to feel empowered towards better gut health. The program aims to build confidence, focusing on strengths as they learn from past failures and successes to overcome current issues around your health.

The total cost of the 4-week taster program is £475 which is being offered free of charge in exchange for feedback and a creation of case studies to be used in the public domain including media. Furthermore, in exchange of the complimentary services, reviews and testimonials will be sought to be used on the Dish Dash Deet’s website, social platforms and media outlets.
